Description:

An exciting opportunity has arisen for 2 x Specialist Clinical Pharmacists working within the South West Wales Cancer Centre at Singleton Hospital. The post holder will be responsible for the day-to-day delivery of pharmacy service to haematology/oncology patients within Swansea Bay area. Pharmacy services include outpatient clinic reviews, non-medical prescribing skills & injectable cancer clinical reviews.
Successful candidates will be appointed to either of the following roles dependant on qualifications:
Advanced Pharmacist – Band 8a (Permanent)
If personal specification is met and experience supports appointment.
Clinical Pharmacist – Band 7 (Secondment 12 months)
The successful candidate with a current level of experience that supports appointment to a clinical pharmacist role but requires further training to support an advanced practice role will be appointed to the band 7 role on a secondment basis for a period of 12 months.
During the training period, they will undertake a series of competencies as well as on the job training. On successful attainment of the competencies to the required level, they will be appointed to the role of Advanced Pharmacist Band 8a. If the competencies are not achieved within 12 months from appointment this will not allow for successful progression to the Band 8a role and therefore the secondment will end on expiry of the 12 month period.
It is an ideal role for a pharmacist wanting experience of providing care to cancer patients & who is keen to step up in a supportive environment. However, specific cancer experience is not needed, & we can provide clinical training to the right candidate.
Your experience will have been gained working in a patient focussed environment using your knowledge and skills to improve patient care & promote optimal outcomes from medicines, including experience of undertaking research and/or audit.
There is a strong focus on learning & development, with the post providing education & clinical leadership within Cancer patients to junior pharmacists, & other health professionals/undergraduates.
In addition, the role will require the ongoing completion of core directorate activities such as reporting & analysis of medicines expenditure, clinical governance & risk management.
Provision & development of an advanced pharmaceutical care service for haematology & oncology patients.
Provide expert advice & guidance on the safe and effective use of medicines & produce of drug related policies & shared care protocols.
Liaison with health professionals in other tertiary centres & in community care to provide on-going pharmaceutical care & advice when patients transfer setting & to ensure continuity of medication supplies.
Investigation of medication related incidents or complaints within haematology / oncology services & explore ways of minimising the risk of future re-occurrence.

This role is a specialist pharmacist in Cancer Services, but will also be required to deputise for the Patient Services Manager/provide cross cover for Clinical Pharmacists on occasions.
