The Wokingham Older Adult Mental Health Service is looking for an Advanced Practitioner and Medication Lead to work primarily within our Memory Clinic. The post holder will be a Non-Medical Prescriber and have highly developed clinical skills and expert knowledge in dementia pathways and older people with mental health problems .
The post holder must be enthusiastic, caring and highly motivated to contribute towards maintaining our standards of high quality care to our patients.
Wokingham Memory Clinic is proud to be an accredited Memory Clinic by the Royal College of Psychiatrists Memory Services National Accreditation Programme (MSNAP). Combined with being an ‘Outstanding’, well-led, organisation, and having an ‘Outstanding’ Older Adults Community Mental Health Team in our latest CQC inspection. The post holder would therefore be expected to work towards helping the service maintain these accolades of high standards.
This post is 24 hours a week – ideally over 3 days, days and times to be agreed.
Responsibilities include: supervising medication reviews, medication side effects and providing high quality psychoeducation to staff as well as patients and carers. In addition, the role will directly supervise the prescription administrator, providing high quality transfer of prescribing into primary care. Furthermore, the post holder will complete initial assessments / feedbacks, and provide duty triage support where required.
Responsibilities:
THE “MUST HAVES” FOR YOU TO BE CONSIDERED FOR THIS ROLE:
- Have the relevant professional qualification ( Mental Health Nurse)
- Hold an active non-medical prescriber qualification.
- You will be an experienced practitioner with the relevant mental health / clinical experience.
- Be a car driver with the ability and readiness to travel.
