SUMMARY:

Under the supervision of Health & Wellness Program Manager, the Health & Wellness Case Manager is responsible for ensuring that Health and Wellness services are provided in a planned, effective and timely manner to eligible clients. The Case Manger provides these services to meet individual client needs that will contribute to the participant’s Health and wellness through outreach, education, and support services. Other duties include representation of the department and Catholic Charities in the San Antonio and Bexar County.

Position Responsibilities:

  • Provides client/clients with an appropriate-culturally relevant reception into the community. This is done through linguistically appropriate interpretation and translation services.
  • Maintain accurate and up to date client’s files, including intake paperwork, assessments and other required paperwork.
  • Provide education and support services to assist clients on how to navigate and access specialized care in their communities.
  • Conducts Interviews to clients and use the input to develop a personalized service plan.
  • Develop and implementation methods to identify those in need of services.
  • Research and Promote Health Awareness campaigns to encourage preventive health.
  • Must be sensitive to the service population’s cultural and socioeconomic characteristic. Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.
  • Prepares daily case file documentation of clients served.
  • Report on activities, meetings, and special client concerns to the program manager and other relevant staff.
  • Communicate concerns regarding client situations or program issues with appropriate staff and come up with a plan of action to address concerns.
  • Follow the agency policies, Health & Wellness Program Standard Operation Procedures, Health & Wellness Manual Guidelines, and directives of Health and Wellness Director as policies are updated or changed.
  • Adhering to safety training and protocols on a daily basis, and taking precautionary measure to ensure the safety and well-being of self, others.
  • Adherence to the Code of Conduct and the Faith and Moral is mandatory.
  • Every employee is required to take a solution-oriented approach in their interactions and undertakings, as well as being a team member that promotes collaboration and commitment to the Mission and Vision of the organization.
  • Other duties as assigned by Program Manager

MINIMUM QUALIFICATIONS:

  • Education /Experience
  • Minimum of two years experience in Medical/ Social Service Feld environment that demonstrates a sensitivity and appreciation for diverse cultural population.

License and Credentials

  • Reliable transportation
  • Valid drivers license
  • Valid vehicle insurance
  • Must be 21 years of age
  • Must have clean driving record
  • Bilingual(Language preference: Pashto or Dari (Farsi)

MINIMUM KNOWLEDGE AND SKILLS:

  • Minimum Knowledge of State and Federal Contracts and Grants.
  • Extensive working knowledge of Program Manuals and Operational Guidelines.
  • Knowledge of client files documentation and record keeping
  • Skilled in using computer, various software, email and internet.
  • Skilled in organization and time management.
  • Must be detail oriented, organized, self-motivated, work well independently and on a team;
  • Must have good written and verbal skills;
  • Must have good critical thinking and problem solving skills.
  • Must have a clean driving record.

TRAVEL REQUIREMENTS:

Travel requirements for the position includes 75% local and 5% overnight.
