Description:
The Alzheimer’s Care Unit Coordinator is expected to oversee the activity functions on the floor and to assure that all federal guidelines are being met for the designated activity program. The Alzheimer’s Care Unit Coordinator is responsible for overseeing compliance with all facility policies and procedures and IDPH/OBRA regulations. The Alzheimer’s Care Unit Coordinator will be the process owner to implement and oversee the Memory Care points of distinction established in the areas of philosophy , environment, programming, family services , dining and leadership.
QUALIFICATIONS
- High school degree or equivalent preferred.
- Certification in accordance with regulatory agencies governing the facility.
- Must have verbal and written communication skills.
- Must have well-developed organizational skills.
- Must have some arts and craft ability and knowledge.
- Must be able to perform record keeping and documentation as necessary.
- Must possess the ability to positively interact with staff, residents, family members, visitors, government agencies, and the general public.
LANGUAGE SKILLS
- Ability to read, write, speak, and understand English.
- Ability to relate positively, effectively and appropriately with patients, families, community members, volunteers and other staff on a daily basis. Possess special interest in, and a positive attitude about, working with long-term care patients and the elderly

Responsibilities:
- Promotion of the facility’s Culture Contract components in all aspects of their role, both for residents, families and staff.
- Implementation and oversight of the Memory Care Services Points of Distinction
- Daily oversight of the day to day operations in the Memory Care Services area in partnership with the Clinical leadership team.
- Oversee the retention, and orientation practices of social service/activities staff that work within the memory care area.
- Evaluation/performance review input provided of all personnel that work within the memory care area by providing input to direct reporting supervisor of C.N.A’s , Nurses, housekeeping, Activities and speaking to the ability to work within the memory care program successfully.
- Management of the Activity program on the designated unit
- Manages the activity documentation system
- Organizes the monthly calendar and needed supplies
- Assists with large group functions/parties
- Management of the medically related social services process on the designated unit
- Manages or directly carries out the social services documentation system, including the concern/grievance process/resolutions
