ABOUT THE ROLE

• To provide coordination of the AN ACC process, to lead, conduct pre-admission and complete a suite of assessments supporting the AN ACC Assessment tools.
• To ensure optimal classification and funding for BaptistCare.
• The position will ensure that BaptistCare pre-admission processes are followed.
• To coordinate and facilitate ongoing review and request for reassessment when resident’s condition has changed significantly.
• To complete any supplemental funding claims to both, maximize and risk manage the home’s government funding, in consultation with Funding Consultant and Client Liaison Team to achieve and maintain budget funding targets.
• To ensure thorough review and audit of Services Plus.

SOME OF THE DUTIES INCLUDE:

• Plan, coordinate and conduct the resident pre-admission.
• Ensure data entered into People Point is accurate.
• Ensure assessments are accurate, appropriate, and timely.
• Review documentation to ensure congruence. Consider both electric and hard copy documentation.
• Compile and follow the AN ACC Checklist as per Framework and Reference Manua
