Pathways Health Partners is an Accountable Care Organization (ACO) at the forefront of helping independent providers make the transition to Value-Based Care. Pathways operates a Medicare REACH ACO, a Medicare Advantage MSO, and a Commercial MSO responsible for approximately 16,000 patients throughout North-West/Central Florida. Our service area is The Villages, down to St. Petersburg, across to Mount Dora, and areas in between. Pathways also operates a robust hospital medicine group, manages several of its affiliated medical practices, and an insurance agency.
Job Summary
The Hospital Medicine Nurse Practitioner (APRN) will engage in direct patient care of hospitalized patients and work with emergency room personnel to determine the best site of care for patients, whether that be inpatient, direct admit to skilled nursing (SNF), or back go home with additional support.

Role and Responsibilities

  • Manages admissions, daily rounding, consultative services, and discharges of hospitalized patients
  • Provides high-quality care of hospitalized patients including medical evaluation, treatment, diagnostics, referrals for subspecialty consultations, and discharge planning
  • Incorporated evidence-based practices in clinical decision-making and provides high-quality care as judged by peers
  • Participates in the multidisciplinary care of the patient
  • Facilitates communication and interactions between the patients, families, peers, ancillary staff, and consultants
  • Ensures accurate and timely completion of medical records, orders, and documentation to maintain compliance with medical staff, local/state/federal, licensure, and accreditation requirements
  • Participates in hospital and ACO quality initiatives and process improvement
  • Ensure efficient use of hospital and other health resources evidenced by reduced length of stay, reduced duplication of services, and reduced readmissions or unplanned admissions below that of non-ACO peers

Qualifications and Education Requirements

  • Graduate of accredited nursing school and board certification culminating in an ARNP/FNP
  • Unencumbered license to practice as an APRN in the State of Florida
  • Credentialing “active” or “admitting” status at facilities in the service area including UF Leesburg Regional Medical Center and UF Health The Villages Hospital (must be in place to begin work)
  • Approved Medicare with PTAN and status with Blue Cross of Florida, United Healthcare, and Cigna (must be in place to begin work)
  • Board-certified nurse practitioner
  • Able to prescribe controlled substances, (DEA license and DCS)
