THKMC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors’ Community, and Therapy.
DESIGNATION : Assistant Case Manager

QUALIFICATIONS

  • At least a Diploma in Social Service, Psychology, Counseling or any other related disciplines

Responsibilities:

  • Adopt a case management approach when working with clients ie. network with other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients
  • Ensure proper documentation of client’s progress and care plans
  • Ensure all clients’ records are appropriately kept
  • In consultation with the Case Manager, provide training and supervision to CAN Carers including recruitment and orientation
  • Communicate, supervise and support CAN Carers
  • Monitor home visit schedule of CAN Carers
  • Participate in outreach activities and events
  • Collect statistics and data relating to service operations
  • Assist with administrative duties relating to the Centre’s operations
  • Any other duties as assigned by the Center Director/ Social Worker / Case Manager
