Responsibilities:

YOUR ROLE

As an Assistant Co-ordinator - Community Care you’ll be looking after the most important people in our organisation - our clients.

Your tasks will include:

  • Assess, plan, co-ordinate and review care delivery
  • Enable clients to maintain as much independence as possible, and encourage them to use their abilities and initiative to develop existing and new skills
  • Report and document changes in behaviour patterns, unusual actions or reactions of clients or employees
  • Provide direct client care when required
  • Assist with scheduling of employees to suit the needs of the client
  • Liaise and network with other service providers and the community as directed

ROLE REQUIREMENTS

  • Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
  • Current Police Screening Certificate
  • Current First Aid and CPR Certificate
  • Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
  • Current Queensland driver’s licence
  • Tertiary Qualifications (as appropriate) and/or relevant experience in a health-related field (e.g. Health and Human services)
  • Experience in case management and client care co-ordination
  • Demonstrated sound verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
  • Flexibility to work across a range of days (Monday to Sunday)
