Description:
- Strategy and workplan
- Support reporting officer in the development of strategy and workplans together with management and RHSO
- Support reporting officer to provide updates on workplan progress to RHSO and NUHS senior management
- Project Management
- Oversee end-to-end project management of assigned collaboration/programme
- Ensure that programme is on track to meet agreed targets/milestones and administrative documents are properly documented and maintained
- Submit timely progress reports as required
- Review programme with internal and external stakeholders for quality improvement
- Partner Engagement
- Identify, prioritise and engage new/potential and existing social care providers and grassroots in the western region to explore and deepen collaborations
- Identify opportunities for strategic collaborations with key partners to improve care integration and augment community care
- Manage regular engagements, feedback and issues from key account community partners
- Keep abreast of developments within social care, community care and key account organisations
- Support implementation of NUHS Population Health strategies and regional health plan
- Keep abreast of NUHS Population Health developments and identify opportunities/gaps/support where appropriate
- Actively sync and identify potential areas of collaboration with NUHS and community to support population health
- Others
