• Strategy and workplan
• Support reporting officer in the development of strategy and workplans together with management and RHSO
• Support reporting officer to provide updates on workplan progress to RHSO and NUHS senior management
• Project Management
• Oversee end-to-end project management of assigned collaboration/programme
• Ensure that programme is on track to meet agreed targets/milestones and administrative documents are properly documented and maintained
• Submit timely progress reports as required
• Review programme with internal and external stakeholders for quality improvement
• Partner Engagement
• Identify, prioritise and engage new/potential and existing social care providers and grassroots in the western region to explore and deepen collaborations
• Identify opportunities for strategic collaborations with key partners to improve care integration and augment community care
• Manage regular engagements, feedback and issues from key account community partners
• Keep abreast of developments within social care, community care and key account organisations
• Support implementation of NUHS Population Health strategies and regional health plan
• Keep abreast of NUHS Population Health developments and identify opportunities/gaps/support where appropriate
• Actively sync and identify potential areas of collaboration with NUHS and community to support population health
• Others
• Any other tasks as assigned
