Description:
We are looking for an enthusiastic, forward thinking Band 4 Associate Practitioner t o join our Pocklington Community Team. This is where you can take your skills out and about as you travel to and from patient homes to provide personalised, quality care in the place where your patient feels most comfortable.
You will part of the UCR (Urgent Care Response Team) covering nursing and therapy needs, you will be supported by a well established team in a lovely rural location of Pocklington.
Service hours are to cover an 8am - 10pm - 7 days per week.
Day to day the role includes the following care carried out in the patient’s own home:
Phlebotomy
Injections
Wound care
Bladder (including catheters) & bowel care
Vital observations
End of life care
Administration and optimisation of medicines
ECG recording
Dopplers
Moving & handling of patients
Participate in audits
Undertake risk assessments
Recognise and escalate issues of concern and/or safety
Therapy support and rehabilitation.
