We are seeking an experienced professional to support our Front door team in Adult Social Care. The ideal candidate will have prior experience in Adult Social Care and be proficient in conducting strength-based screening assessments in compliance with the Care Act 2014. Additionally, the ability to work independently and preferably possess experience in utilising Mosaic (with the option for training) is highly desirable.
The primary responsibilities of this role include conducting screening assessments to evaluate the potential needs of our residents, identifying the necessity for social work and/or occupational therapy intervention, and escalating cases to case management when necessary. The successful candidate will also provide information and advice to promote self-sufficiency and connect residents to community resources. Furthermore, they will be responsible for self-allocating work and providing case updates to the Deputy team manager.
The Front door team in Adult Social Care in Southwark is a busy, first-line team that strives to empower and connect residents using strength-based approaches, promote self-resilience, and prevent long-term support needs. The team is currently composed of Customer Support Officers, an Occupational Therapy Assistant Practitioner, three Occupational Therapists, three Social Work Deputy Team Managers, and a Social Work Team Manager.
The position offers flexibility, allowing for two days of remote work per week. The initial contract is for three months, with the possibility of an extension based on the services needs.
