For full details please refer to Job Description and Person Specification To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements. To ensure continuity of care by: Providing client with contact numbers Planning appointments Maintaining contact during episodes of in-patient care and involvement in discharge planning Contact and communication with GPs & other Primary care staff Out of hours arrangements/cover for Annual Leave To manage the transfer of a clients care to another team member or service Attend appropriate clinical meetings either in the community or other settings in relation to the clients care. Liaise with a wide range of other professionals and agencies. Provide treatments and activities in a range of venues including the persons home Contribute occupational therapy skills to the multi-disciplinary team.
Monitor and evaluate the therapeutic value of OT individual and group work interventions and contribute to evaluation and audit of the service as a whole. Support the development of the peer support workers and ensure they are equally included in design and delivery of relevant aspects of the service and promote co-productio
