Description:

An opportunity has arisen in Swindon for an enthusiastic and experienced nurse, Social Worker, OT or other suitably registered person to work as a Band 6 Specialist Intensive Practitioner. The successful applicant will be working at Sandalwood Court, Swindon. The multidisciplinary team is friendly, supportive and well established. Staff share a caseload of people who need intensive support.
Key roles include, leading shifts as a shift coordinator, reviewing and accepting referrals, completing thorough core assessments, risk management and care plans, with the intention to treat people at home in the community if possible, or for gatekeeping if a hospital admission is indicated whilst ensuring we use the least restrictive practice at every opportunity. The service also supports people leaving hospital prior to referring or signposting or to other AWP or voluntary and third sector agencies. The successful applicant will also contribute to ensuring the safety of our service user’s, carers and others by taking part in a variety of multidisciplinary meetings. There will also be an opportunity to contribute to Service development and lead on new ways of working as the team change in line with CMHF.
Accepting and reviewing referrals, completion of assessments within the community and general hospital settings for home treatment or gatekeeping for hospital admission. Implementation of Risk Management and Care Plans for the effective home treatment of Service Users utilising least restrictive practice at every opportunity. Medication management and concordance within community. Other duties and responsibilities including inline with job description.
There will be a comprehensive induction to the role and a supported period of supernumerary to aid with transition.
Swindon Intensive Service (SIS)
SIS is a 7 day a week 8am-10pm service and the successful applicant will be able to work a mixture of shifts that can enrich one’s home life/work balance. Enhancements are paid for unsocial hours.
Excellent communication and organisational skills as well as an ability to work with a diverse staff group and autonomously are essential to this role, along with the clinical knowledge and skills such as an in-depth knowledge of safeguarding when working at the level of a Band 6 member of staff. There will be a comprehensive induction to the role and the successful applicant will have a supernumerary period to aid with the transition. Regular meaningful supervision and opportunities to enhance and learn new skills is available and encouraged.

To be responsible for developing delivering and reviewing comprehensive and complex treatment programmes using appropriate frameworks in line with evidence–based practice, including strategies to manage risk for service users with complex needs and carers, bringing in other resources as required.
To provide defined interventions to individuals on other team caseload. Team Manager, Senior Practitioner, Band 6 Specialist Intensive Practitioners, Band 5 Intensive, Practitioner, Band 4 Intensive Recovery Co-ordinators, medical staff and administrators.
To plan, deliver and evaluate defined, specialist therapeutic interventions as indicated in the assessment process ,in line with personal recovery plans, including to service users who maybe on other caseloads.
