As a team we share in the vision home first, this approach acknowledges that often patients needs are best managed in their own environments, this is often also the wishes of the patient. We are key to identifying risks and barriers to this and finding solutions with the collaborative support of the multidisciplinary team. We have established links and regularly network with our primary care services, social services and external agencies within the ICS to make this process from hospital to home as streamline as possible. You will be supported by our Occupational Therapy team leader with yearly appraisal and regular formal and informal supervision.
In addition to attending and contributing to targeted IST within the ED and Frailty therapy team you will be invited to the established therapy department weekly in-service training.
