Description:

POSITION SUMMARY:

This employee provides individual psychotherapy, specialized group counseling, individual and group
family therapy, family outreach and care management services to adults with Substance Use
Disorders (SUD) and/or co-occurring Mental Health Disorders and SUD, so they may achieve stable
mental health and sustained remission from SUD, restore and increase healthy social connections,
obtain stable income and permanent housing, and reduce their risk of harm, hospitalization,
incarceration, and homelessness . The employee also conducts diagnostic assessments and provides
consultation to Case Counselors serving clients with co-occurring conditions.

EXPERIENCE:

Must have at least 5 years’ experience in behavioral health counseling and psychotherapy
and/or Family Therapy.

TECHNICAL SKILLS AND KNOWLEDGE:

Proven ability to meet minimum productivity requirements (five hours per day of direct reimbursable clinical care).
Must have successful experience facilitating behavioral health group counseling.
Preferred License: LISW (Licensed Independent Social Worker) or LPCC (Licensed
Professional Clinical Counselor).

Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population.
Must have basic computer skills, using Microsoft Office applications.
Should have working knowledge of OHMAS, CARF and VA program compliance standards.

Responsibilities:

Provide individual psychotherapy, specialized group counseling and mental health care
management to adults with co-occurring Mental Health and Substance Use Disorders.
Facilitate specialized trauma informed group counseling and psychoeducation
programming.
Provide individual family needs assessments, individual family therapy, and group family
education.
Facilitate specialized group education on the role of families in SUD recovery. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.
Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for mental healthcare and vocational rehabilitation.
Participate in team meetings, in-service trainings and coordination meetings.
Coordinate with Clinical Director on all critical clinical issues.
Conduct Diagnostic Assessments.
Provide mentoring and support to Case Counselors regarding mental health, family, and intimate relationship matters.
Other duties as assigned.

Required Education: Master’s Degree in relevant field.
