Description:

JOB SUMMARY: Responsible for delivering case management, referral, benefits navigation services for clients and providing outreach to the community.

Responsibilities:

  • Designs, implements and monitors individual program plans for clients.
  • Assesses client needs; investigate their eligibility for services.
  • Counsels and educates individuals and families requiring assistance.
  • Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
  • Coordinates client activities.
  • Assists clients with accessing benefits and services (application assistance, service referral, community partner advocacy, etc.)
  • Networks with community providers to generate community resources for participants and referrals to program.
  • Presentation to consumer and community partners regarding benefit access and services.
  • May transport consumers to appointments or meet with consumer/partners in the field.
