New York Breast Health, a prominent and respected Breast Health group, is seeking a Breast Health Nurse Navigator to join a well-established and growing pure sub-specialty practice with academic affiliation. NYBH is the leading surgical oncology group specializing in diseases of the breast.

• Work with the interdisciplinary team to execute a customized plan of care for each unique patient.
• Guide the patient through the treatment process, reviewing instructions, and scheduling appointments.
• Facilitate interactions between patients and their physician and referring physician(s) as appropriate.
• Navigate the patient and family across care settings according to the established patient care plan and clinical pathways
• Utilize effective communication to ensure patient and caregiver(s) have appropriate information, understand, and have the capacity and tools to adhere to the individualized treatment plan.
• Contribute to quality-driven performance improvement initiatives.
