Description:

A rare opportunity has arisen to work within our Cardiac Rehabilitation Specialist Team at the Conquest Hospital and Eastbourne District General hospital.
We are looking to appoint a self-motivated, enthusiastic and positive thinking nurse; to work in a cross-site role. To work 30 hours a week over 4 days - 2 days at each hospital site. This person will need to have a keen interest in cardiac prevention and rehabilitation, and be able to practice autonomously.
This is an ideal opportunity for anyone interested in this specialist area of cardio-vascular medicine to develop expertise and further his or her professional development.
You will be responsible for an individual caseload of clients post referral through to discharge from the programme, this will cover in-patient and out- patient settings, clinics, and exercise groups.
The role involves careful and comprehensive clinical assessment throughout the patient’s treatment; the safe running of and participation in the out- patient exercise groups and cardiovascular risk assessment clinics. It includes advice on optimising secondary prevention. You will provide individualised, cardiovascular risk assessment, lifestyle and risk factor modification, and behaviour change advice.
You should have experience in the cardio-vascular field with good interpersonal and presentation skills. You will need to feel confident to independently manage a patient caseload, including care planning and co-ordinating their menu-based rehabilitation programme. You will work well within the team to provide and supervise a comprehensive cardiac rehabilitation programme from referral to discharge, adapted to individual clinical need and patient preference, for all cardio-vascular patients meeting the referral inclusion criteria.
We are looking for someone who demonstrates particular skills and interest in cardiac rehabilitation; who is a registered nurse with at least five years post registration experience, some within cardio-vascular medicine and who demonstrates drive and excellent communication skills.
In return, you will be part of a very friendly, experienced and supportive team, dedicated to helping you to develop your career in this clinical field. We welcome the expertise that you will also bring. If you have not yet worked within cardiac rehabilitation but you are very interested in the role, do not hesitate to call us for further discussion. Further professional training and updates in the specialty to allow you to fulfil your role and develop expertise will be provided.
We encourage all staff to participate in Continuing Professional Development.
