THE OPPORTUNITY
For this dual role, we are seeking adaptable, flexible, passionate and experienced RNs to work on road as a Community Registered Nurse for our Hospital and Rehab in the Home (HITH and RITH) program and also contributing to the Rehab in The Home (RITH) program as a Care Coordinator.
As a Care Coordinator, working from home, you will organise initial assessment and coordinate patients’ treatment plans and educate them about their condition as well as connect them with relevant health care provider; check in and evaluate their progress and goals and upon completion of program manage discharge admin and reporting.
As a Community Nurse, in Toowoomba, you will build rapport with your clients in their homes, whilst providing care and service to clients in line with their personalised care plans, including health assessments, chronic wound care and medication administration as well as providing education and support to the patient and their family.
This is a permanent position with a minimum of 0.8 FTE. Your training will begin with the Care Coordinator role and after about 3 months you will start your training for the Community Nurse role. Once trained, you will work a 7 day rotating roster, working 2 days as the Care Coordinator (work from home) and 2/3 days as a Community Nurse (on the road).
