Description:

Responsibilities:
Responsible as a member of the treatment team for coordinating all Recovery Management throughout the continuum of care, directly working with patients coordinating the development of the continuing care plan. Assist in providing all clients a smooth transition through the Hazelden Betty Ford continuum of care, effectively communicating to families, referents and clients about recovery management.

As a Care Coordinator, you will:

  • Assist clinical case managers by working with clients, gathering, compiling and coordinating continuing care information
  • Provide AA/NA contacts in clients home area, set up interviews, arrange transportation and contact agencies with regard to clients continuing care plan
  • Maintain the continuing care database, with special emphasis on continuous updates of clinical value and work
  • Work with the admissions office and provide back up for admitting clients, when needed, as well as provide the continuing care chart checks for all patients leaving treatment
  • Follow up phone calls within 5 days of discharge for WI client facility placements for continuing care services.

Qualifications:

Required

  • High school diploma or equivalent
  • 2 or more years in customer service
  • Intermediate level experience with Word software, including Outlook, Word, PowerPoint
  • Experience in online research
  • Current CPR certification.
  • Certification must be from American Red Cross or American Heart Association.

Preferred

  • Vocational or associates degree
  • Experience with patient database
  • Basic knowledge of chemical dependency, including 12 step programs and the MN model
  • Experience in a behavioral care setting

Min: USD $21.59/Hr. Max: USD $30.48/Hr. Overview:
