Right at Home provides high quality in-home care and assistance to our valued clients in the Sunshine Coast, Gympie and Moreton Bay regions. Our mission is to improve the quality of life for those we serve. We provide care to those in aged care, post-operative care and people living with disabilities. Our services help clients remain safe and independent in their own homes.
About the role
We are expanding our team and seeking to recruit dedicated individuals who strive for best practice, are able to communicate well with people of all backgrounds and approach every situation with care and compassion. The Care Coordinator will be responsible for:

  • Building strong connections with clients and their families
  • Assessing client needs and resources for health, welfare and other facilities and services
  • Conducting Client Care Assessments and Care Planning
  • Maintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework
  • Supporting all clients to meet their goals
  • Supporting and providing feedback to Home Care Workers
  • Re-organising work priorities to meet the needs of a fast-paced environment
  • Must promote the values of the company and prepared to plan supports using a contemporary approach

Essential Criteria:

  • Experienced in assessing and planning care needs
  • Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework
  • Organisational, administrative and time management skills
  • All candidates must have the right to work in Australia
  • COVID 19 Vaccinations
  • Registered Nurse or Enrolled Nurse (Desirable not essential)

Ability to Obtain:

  • National Police Check
  • Blue Card
  • Drivers Licence
  • NDIS Worker Screening Check
  • First Aid & CPR Certificate
