DESCRIPTION

We are looking for an experienced Care Manager who is qualified, reliable, and passionate. with planning and organizing skills to meet both client and business needs. We are seeking an experienced, enthusiastic, and highly motivated Care manager to coordinate, manage and administer the delivery of care services to our Chinese speaking clients. Our clients are a mix of Private, Home Care Package and NDIS across all age groups. Care manager must be willing but not limited to:

  • Dealing with a range of clients with disabilities, seniors and frail elderly customers and their families on a daily basis, this role will handle anything from information requests, feedback, general enquiries to complaints.
  • Serves as a liaison between referral sources, families, carers and administrative staff members.
  • Ensure all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home to ensure a positive and professional client experience every time.
  • Use a variety of superior interpersonal communication skills to deliver genuinely respectful client engagement including active listening, gaining all customer details, information gathering, problem analysis and solving, persuasiveness, and negotiation.
  • Provides client and family education on the nature of services that are available and the effect and benefit of the care plan.
  • Prepares in-home care assessments and drafts care plans to best meet client requirements.
  • Performs the duties of a nurse as needed and monitors the quality of client care.
  • Engages in studies and investigations related to improving personal and nursing care.
  • Approves the supply of medical devices and aids as required by clients from time to time. Negotiation with approved suppliers
  • Delivering outcomes/goals for our clients based on their care/NDIS plan.
  • Collaborating with our clients to coordinate support and services to improve their quality of life.
  • Conducting client care assessments and care planning with a person-centered care approach in alignment with goals/outcomes and within budget constraints.
  • Understanding, managing and working within government contractual requirements.
  • Supervision and management of Care Staff.
  • Schedule appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements with our unique carer matching at the forefront of decisions.
  • Coordinate the provision of quality person centered care according to the client’s needs, service/care plans and available funding in accordance with budgets
  • Building relationships with local stakeholders
  • Positive attitude with the ability to work in an ever-changing environment.

QUALIFICATIONS

  • Current AHPRA Registration
  • Relevant tertiary qualification (Aged Care, Management) – Desirable
  • Certificate in a specific nursing related discipline (Dementia care, geriatric nursing, palliative care) - Desirable

PREFERRED SKILLS

  • Meets the competency levels for the Registered Nurse as set out by the Australian Nursing & Midwifery Council (ANC) guidelines, and particular organizational requirements. Community Care Common Standards & the Community Care Charter of Rights and Responsibilities.
  • Fulfils the obligations of the Nursing Role for the Registered Nurse including Code of Ethics and Code of Conduct
  • Establishes and maintains effective communication and harmonious working relationships.
  • Updates knowledge and skills by pursuing continuing professional development.
  • Time management and organizational skills
  • Ability to develop/implement and evaluate organizational and client focused documentation and systems.
  • Analytical and report writing skills
  • Ability to create and promote a positive and supportive culture where all staff are valued.
