JOB SUMMARY:

The Care Navigator:

  • Assists members in accessing the right care at the right time by building strong relationships that support member engagement in their medical and/or behavioral health care treatment plans.
  • Is committed to removing the member’s barriers to care by identifying needed resources, helping them navigate through healthcare services and systems, promoting holistic health through education, and reducing social barriers. The Care Navigator helps members to optimize the use of their benefit plan through steerage to in-network providers and work closely with the Clinical Care Team, which may include but not be limited to doctors, nurses, and other clinical staff to support positive client health outcomes.

REQUIRED QUALIFICATIONS/EXPERIENCE:

  • Bachelor’s degree in a health-related field (e.g., Social Work, Counseling, Psychology, or another closely related healthcare field) with 1+ years of experience in medical, mental health, and/or behavioral healthcare, social work, community outreach, or patient advocacy.
  • 2+ years of experience in medical, physical health, mental health, and/or behavioral healthcare, social work, community outreach, or patient advocacy in a clinical field, remote, or onsite environment.
  • Understanding or experience in completing health or social assessments.
  • Ability to demonstrate purposeful and outcomes-directed critical thinking within daily operational activities.
  • Must be comfortable educating members and developing an individualized plan of care.
  • Must be comfortable working telephonically and with computer systems while assisting members.
  • Must be able and comfortable with maintaining metrics and goals.
  • Must be comfortable providing outreach to members.
  • Excellent verbal/written communication skills.
  • Ability to work in a team environment.
  • Flexibility and strong organizational skills.
  • Must be proficient in MS Office and Internet/Web navigation.

PREFERRED QUALIFICATIONS/EXPERIENCE:

  • Master’s degree in Social Work, Counseling, Psychology, or another closely related healthcare field with 1+ years of experience in medical, mental health, and/or behavioral healthcare, social work, community outreach, or patient advocacy.
  • Licensing: LCSW, LCPC, LPCC, or LMFT
  • 3+ years of experience with member outreach and health assessment integration into person-centered care plans.
  • Strong understanding of cultural competency with the working population.
  • Experience with commercial health plans.
  • Bilingual (English/Spanish).

Responsibilities:

JOB RESPONSIBILITIES:

  • Outreach to members to assist them in meeting their preventative health needs.
  • Complete health and social needs assessment to assist in the development of individualized care plans.
  • Work within a patient’s benefit network for the best outcome and to enhance access to care.
  • Potentially cross-trains to support other contracts within Acentra Health to provide a flexible workforce and meet client/customer needs.
