EXPERIENCE & EDUCATION REQUIRED

CMHA Toronto’s ideal candidate will have an undergraduate degree or diploma in social services or related field. The candidate will bring a minimum of three years experience in direct service delivery to adults with serious psychiatric disabilities, experience in community mental health will be considered an asset. The Candidate will have sound knowledge of crisis intervention/prevention, addiction issues, recovery-oriented approaches, culturally competent care, trauma informed care, as well as severe and persistent mental illness.
The candidate will bring a solution focused approach to work and the ability to manage crisis, as well as ability to work with diverse communities and demonstrate a non-homophobic and anti-racism approach. The candidate will also have the ability to work within a multidisciplinary team as well as work independently and possess effective analytical, assessment, and decision-making skills.

Responsibilities:

THE ROLE AND OPPORTUNITY

The Canadian Mental Health Association is looking for a Case Manager for the Community Treatment Order Program, East Team (CTO). The Community Treatment Order Program is an intensive case management program serving people with severe mental illness who have been issued a CTO. The program’s philosophy is based on psychosocial rehabilitation principles that emphasize recovery through consumer participation and collaboration. Services are targeted to people who are sixteen years of age and older, have a severe mental illness, have been issued a CTO, and require support to remain in their community. The successful applicant will work on the East CTO Team.

RESPONSIBILITIES

Reporting to the Program Manager, the Case Manager will work with the client, CTO Coordinator, treatment team, Substitute Decision Maker (if any) and relevant others in the development of a plan and provide case management for the duration of the CTO. The Case Manager will assist clients to meet the obligations of the CTO and access and link with other appropriate services as necessary.
The role will involve conducting client assessment/individual service planning and develop therapeutic alliance with clients and their support systems. Further, the Case Manager will build and maintain a trusting respectful and collaborative working relationship with client and be able to provide supportive counseling for a range of issues, skills teaching as well as psycho-education around illness, medications and symptoms.
This role will involve direct service provision/intervention with a strength-based approach. The Case Manager will also develop, implement and monitor the service plan and conduct follow-ups with the clients as needed. The Case Manager will advocate on behalf of the client and connect them with community resources as well as establishing effective working relationships with staff of other agencies.
