EXPERIENCE & EDUCATION REQUIRED

CMHA Toronto’s ideal candidate will have an undergraduate degree or diploma in social services or related field. The candidate will bring a minimum of three (3) years experience in direct service delivery to adults with serious psychiatric disabilities, experience in community mental health will be considered an asset. The Candidate will have a sound knowledge of crisis intervention/prevention, addiction issues, recovery-oriented approaches, trauma informed care, as well as severe and persistent mental illness
The candidate will bring a solution focused approach to work and the ability work with diverse communities and demonstrate a non- homophobic and anti- racism approach. The candidate will have excellent communication, listening and interpersonal skills and have the ability to work within a multidisciplinary team as well as work independently

Responsibilities:

THE ROLE AND OPPORTUNITY

The Canadian Mental Health Association – Toronto is looking for a Case Manager for our East Case Management Program. This Case Management program offers intensive case management services as part of the continuum of mental health services and supports for people with mental illness. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the client. The direct involvement of the client and the development of a caring, supportive relationship between the case manager and the client are integral components of the service. Service is provided in the community.

RESPONSIBILITIES

Reporting to the Program Manager, the Case Manager will work with the interdisciplinary team to support their clients. They will be required to participate in team meetings and provide regular updates to managers.
The aim of the Case Manager will be to assist in the development of a service care plan, access appropriate services and provide case management for clients accepted to the case management service. This will begin with intake of client information to assess their needs and identifying the supports required, this will be done through effective consultation and collaboration with the client.
This role will also involve direct service provision/intervention with a strength-based approach. The Case Manager will also develop, implement and monitor the service plan and conduct follow-ups with the clients as needed. The Case Manager will advocate on behalf of the client and connect them with community resources as well as establishing effective working relationships with staff of other agencies.
