Description:

Case Manager, Senior
The Senior Case Manager assists the DCM with the coordination and management of day-to-day operations of the Case Management department including, but not limited to case manager schedule and assignments, KCI / outcomes monitoring, medical record reviews, performance improvement activities, monitoring adherence to standards of Care Management and Readmission Prevention programs, and utilization review process. The senior case manager provides training and mentoring for new case managers and students and serves as a subject matter expert and resource to case managers in the department and the interdisciplinary team. The senior case manager serves as department supervisor in the absence of the hospital DCM or Senior Certified DCM. The Senior Case Manager is assigned a patient case load consistent with hospital operational needs taking into consideration additional duties and tasks being performed.

QUALIFICATIONS

License or Certification:

  • CCM or ACM certification required.
  • Registered Nurse (RN:
  • Must hold a valid, unencumbered license in state where the hospital is located or from an appropriate Nursing Licensure Compact state.
  • Licensed or certified health care professional other than RN:
  • If licensure is available within the state where the hospital is located and is applicable to the individual’s educational level (e.g., BSW), the individual must hold a valid, active, unencumbered license.

Minimum Qualifications:

  • Registered Nurse (RN):
  • Minimum of associate degree in nursing (AND or ASN) required. Bachelor’s degree (BSN) preferred.
  • A diploma in nursing is acceptable only in those states whose minimum requirement for RN licensure is a diploma rather than an associate degree.
  • Licensed or certified health care professional other than RN:
  • Minimum of bachelor’s degree in a health or human services-related field. Master’s degree preferred.
  • Minimum 2 years of experience in Case Management, including Utilization Review and Discharge Planning in an inpatient rehabilitation setting.

Machines, Equipment Used:

  • General office equipment such as computer/laptop, telephone, copy/fax machine, calculator, scanner, etc.
  • Microsoft Office software, to include Outlook, Word, and Excel.

Physical Requirements:

  • Visual acuity, speech recognition, speech clarity.
  • Ability to lift, lower, push, pull, and retrieve objects weighing a minimum of 30 pounds including medical supplies and equipment and the transferring and repositioning of patients. Reasonable assistance may be requested when lifting, pushing, and/or pulling are undertaken which exceeds these minimum requirements.

Skills and Abilities:

  • Oral communication, written communication, active listening. Must be able to speak and understand English.
  • Information ordering, deductive reasoning, social perceptiveness, time management, critical thinking.
  • Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner.
  • Ability to work independently without continuous supervision.
