We are currently seeking a dedicated Case Manager to join our company and oversee our team of passionate caregivers. As a Case Manager, you will play a pivotal role in ensuring the delivery of exceptional care services to our clients while managing and coordinating our team of caregivers.

Responsibilities:

Caregiver Management: Train and supervise a team of caregivers, ensuring they provide high-quality care services to our clients. Conduct regular performance evaluations and provide ongoing support and guidance to ensure continuous improvement.
Care Coordination and Planning: Collaborate with clients, their families, and healthcare professionals to develop personalized care plans. Coordinate and communicate care plans effectively with caregivers, monitoring client progress and adjusting plans as needed. Ensure that all care services are delivered in compliance with established protocols and industry best practices.
Quality Assurance and Compliance: Maintain compliance with regulatory standards and company policies. Conduct regular audits to assess caregiver compliance with care plans and company standards. Investigate and resolve client and caregiver concerns promptly and efficiently. Stay updated on industry trends and advancements to ensure the highest quality of care services.
Client and Family Support: Provide ongoing education, support, and guidance to clients and their families. Act as a liaison between clients, caregivers, and healthcare professionals, facilitating effective communication and collaboration. Address client and family questions or concerns promptly and professionally.
