Selection Criteria:

Essential

  • Experience as a Case Manager in Aged care or Disability preferably with HCP funded clients.
  • Relevant qualification in community service, or equivalent.
  • Demonstrable commitment to quality client care and services.
  • Highly developed interpersonal and communication skills.
  • Solid time management, organisation and prioritisation skills.
  • Ability to problem solve issues that may arise in a home care environment.
  • Proficient in MS Office, client management systems and technologically capable.

Preferred

  • Significant knowledge of and experience working with HCP funded clients.

ABOUT THE ROLE:

Reporting to the Care Services Manager and working day to day with other Care Coordinators (our name for Case Managers), Schedulers, Support Workers and Registered Nurses, this role has overall responsibility for managing the care and services of a designated list of Astley Care’s HCP clients. The position will support our HCP clients to live in the community with choice, dignity and independence.

DUTIES WILL INCLUDE:

  • Maintain an excellent knowledge of legislation, guidelines and information relevant to service provision and funding, and ensure services and supports are provided within all boundaries.
  • Undertake comprehensive assessments of client needs, identify potential risks, and make recommendations for care and services.
  • Develop and monitor agreed client budgets, ensuring all services and supports are reflected, and ensuring the client understands subsidies, supplements, costs and contributions.
  • Develop and maintain a current person-centred strength-based Care Plan for all clients, reflecting services and supports, identifying goals and strategies to achieve them, and providing direction to care staff.
  • Identify when referral to other agencies/health professionals is appropriate and action within agreed timeframes.
  • Provide ongoing support and advice, and act as first point of contact to clients, significant others, and external providers. Advocate for vulnerable clients to ensure their safety and wellbeing.
  • Be willing to promote Aged Care to community groups.
  • Participate in out of hours on-call rotation as required.
