Description:

WHO IS A CASE MANAGER, HOUSING FIRST FOR YOUTH?

The Housing First for Youth (HF4Y) program is designed to address the needs of developing adolescents and young adults by providing them with immediate access to housing that is safe, affordable and appropriate, and the necessary and age-appropriate supports that focus on health, well-being, life skills, engagement in education and employment, and social inclusion. The goal of HF4Y is not simply to provide housing stability, but to support and facilitate a healthy transition to adulthood.
As the successful candidate, you will be responsible for providing intensive Case Management services and support to a small caseload of youth (7-10 maximum) that are experiencing, or at significant risk of, being unhoused. Following a fidelity based HF4Y program model, you will support young people in finding and maintaining housing, connecting them with necessary and complimentary professional and natural supports, with the goal of ending their experience of homelessness, preventing future episodes of homelessness, supporting recovery and connection to community. Services will take place in the community and in the young person’s home. This is a permanent, 37.5 hour per week position based out of Foundry Kelowna.

YOUR STRENGTHS & EXPERIENCE:

  • Valid class V BC driver’s license, reliable vehicle for business purposes and acquire liability insurance
  • Minimum Bachelor’s Degree in a Social Services or related field, or equivalent combination of education, training, and experience
  • Minimum 2 years of experience working with youth and young adults, including experience with Case Management
  • Knowledge of current best practices and approaches in preventing and ending youth homelessness
  • Knowledge of health and social service systems in both the youth and adult system of care.
  • Experience working with young people with serious mental illness and/or addictions and those that have experienced homelessness
  • Ability to multi-task in a fast-paced environment.
  • Computer skills
  • Written and verbal communication skills
  • Negotiation/communication/crisis management skills
  • An ability to manage a flexible work schedule
  • Current first aid certification
  • Full vaccination against COVID19

Responsibilities:

  • Collaborate with participants in creating a comprehensive plan of care unique to each person and reviewed and/or modified regularly;
  • Provide case management, outreach, advocacy and access to information and other services;
  • Provide services that address the unique needs of youth, such as developing life skills, navigating systems and services, integrating mental health and addictions supports;
  • Maintain written and computerized records, including case notes, data/statistical information, and other relevant documentation;
  • Inform and advocate for youth to access community resources, benefits, and services applicable to them;
  • Provide an ethical, efficient and therapeutic approach to dealing with crisis intervention in a timely manner;
  • Collaborate with staff and community partners at Foundry Kelowna;
  • Build and maintain partnerships/relationships across the community;
  • Meet regularly with the Community Roots Manager;
  • Other duties as required.
