Description:

JOB REQUIREMENTS:

  • Education /Background: Minimum of a High School Diploma or equivalent with at least 2 years of relevant experience. Bachelor’s Degree in social work preferred. Ability to speak, read and write a language of one of the refugee populations served preferred.
  • Job Knowledge: Must possess interviewing and case recording skills; must be familiar with American local laws, customs and cultures. Must possess community service resource knowledge. Must be able to work under pressure and be self-sufficient in work duties. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications (Word & Outlook).
  • Working Conditions/Physical Demands: Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. Requires work outside of the office and driving; requires night and weekend work on occasion and willingness to be on-call for emergencies; may lift up to 25 pounds.
  • Other requirements:
  • Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
  • You must be 21 years of age or older to drive on behalf of Catholic Charities.
  • Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.

Responsibilities:

JOB DUTIES:

  • Conducts an intake and assessment with each potential client. Reviews client documentation and determines program eligibility, provides client with an in-depth orientation on program guidelines and requirements, obtains relevant consents for service; reviews client rights and responsibilities, grievance process, protected health information, and any other required program documents.
  • Provides intensive case management services by working in collaboration with departmental staff; Works directly with clients in exploring client’s perception of problems, strengths and weaknesses; develops short and long term goals to increase self sufficiency; coordinates referrals to appropriate resources/programs.
  • Maintains client case file with required forms and documentation; completes case note recordings no later than 2 business days after each client contact. Ensures relevant forms, case plans are completed within program requirements and updated to demonstrate client progress towards goals.
  • Instructs refugees in the management of an American home and in American economics including TANF benefits, managing household budgets, direction in parenting skills, alerting clients to state laws regarding child abuse and neglect.
  • Assists clients in health maintenance by making appointments to determine eligibility for medical assistance programs; helping client obtain needed medical care and medication.
  • Ensures that services are provided in a culturally and linguistically appropriate manner and will ensure/advocate for interpretation is provided by service providers when they are Title VI mandated.
  • Explores community resources and makes resources available to clients by developing and maintaining good relations with community agencies; acting as liaison for refugees needing services from other agencies and organizations.
  • Maintains case records by recording each contact with clients in case record; preserves the confidential nature of the files; observes departmental guidelines regarding maintenance of documentation and records.
  • Develops and maintains statistics, correspondence and reports by keeping monthly statistics, completing monthly staff activities reports, answering correspondence and generating information and reports as needed or assigned.
  • Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
  • Performs other duties as assigned.

THIS POSITION WILL BE 90% IN OFFICE IN ORDER TO SUPPORT CLIENT NEEDS AND COMPLETE ADMINISTRATIVE TASKS. A SMALL PORTION MAY BE WFH WITH SUPERVISOR APPROVAL, BASED ON PROGRAM NEEDS.
