EDUCATION AND EXPERIENCE:

  • Bachelor’s degree in mental health, counseling, social work, human services, or a related field
  • For social work degrees, social work licensure in the appropriate jurisdiction is required.
  • For professional counseling degrees, professional counseling licensure in the appropriate jurisdiction is required.
  • One (1) year of experience working as a case manager in the social services field.

SKILLS AND COMPETENCIES:

  • Ability to listen actively, communicate effectively, and convey points clearly in written and oral form. Organizational and time-management skills, including the ability to be flexible and prioritize tasks.
  • Customer service skills, including the ability to calm frustrated clients and deal with clients in a professional, friendly, and patient manner.
  • Skill in the use of computers, preferably a PC, Windows-based operating environment, Microsoft Office products, cloud environment, and databases

Responsibilities:

  • Interview clients for direct service needs and develop a case plan with the client including written short-term and long-term goals and tasks to achieve housing stability. Make referrals as needed.
  • Conduct ongoing assessments of client needs and reevaluate goals with clients at regular intervals. Deliver and coordinate services to individuals and families and maintain direct contact.
  • Facilitate daily crisis management, acting as a liaison between client and client’s environment.
  • Collaborate with other disciplines including substance abuse, legal, medical, educational, vocational, and mental health professionals, and other service providers.
  • Make appropriate referrals for emotional, substance abuse, legal, medical, dental, educational recreational, vocational, employment, and housing needs.
  • Create and maintain case records, including, but not limited to legal documentation, assessments, case notes, case plans, service updates, reports, medical reports, referrals, discharge summaries, visitation records, and educational information in designated databases such as the Homeless Maintenance Information System (HMIS), QuickBase and STEP within contract requirement timelines.
  • Enhance the client environment by creating a hospitable and customer-oriented facility.
  • Complete reports and maintain statistical data, meet programmatic, documentation, and other performance standards, on an ongoing basis.
  • Perform other job-related duties as assigned.
