Description:

Under immediate to general supervision, the Special Services Docket Case Manager represents The Association at the Special Services Docket. This position provides services such as advocacy, connecting participants with resources and referring participants to a variety of behavioral health and community settings and performs a wide range of tasks to assist participants in the recovery processes.

KNOWLEDGE, SKILLS & ABILITIES:

  • High School Diploma or GED required.
  • PRSS Certification preferred.
  • Demonstrated participation in advocacy and/or personal knowledge in recovery.
  • Be willing to self-disclose about personal recovery and wellness.
  • Be stabilized in recovery and practice ongoing self-care.
  • Be able to serve as an example for others of what wellness and recovery looks like.
  • If PRSS Certified, agree to respect and observe the PRSS Code of Ethics.

Responsibilities:

  • Conduct a comprehensive individual assessment and participant planning using a trauma informed approach and motivational interviewing skills. Identify participant’s preferences, goals, strengths and needs related to housing, physical/behavioral health issues, independent living skills and activities of daily living including nutrition and diet, leisure interests, aptitudes, social inclusion.
  • Monitor participant status, personal plan, and service/supports provisions to determine plan effectiveness. Evaluate and modify personal plan to meet participant’s changing behavioral health and other service needs. Make recommendations on compliance with case management requirements for Special Services Docket program.
  • Share knowledge about and resources for various behavioral health settings and ancillary services (i.e. Social Security, housing services, supportive employment, and advocacy organizations) and coach on effective coping techniques and self-help strategies.
  • Maintain a working knowledge of current trends and developments in the behavioral health field through ongoing participation in training and other educational opportunities.
  • Provide and advocate for effective recovery-oriented services as needed and attend treatment team and program development meetings to ensure the presence of the participant voice and to promote the use of self-directed recovery tools.
  • Empower participants through promotion autonomy, self-direction, and self-advocacy as they near graduation from the Special Services Docket program.
  • Provide regular updates and communication with the court system regarding the Special Services Docket.
  • Maintain participant records (documents, progress notes, treatment plans, etc.) in various systems including the Homeless Management Information System and internal data management software.
  • Perform various administrative functions to assist the court and other service providers in preparing the docket and facilitating court proceedings.
  • Performs other duties as assigned.
