Responsibilities:

ABOUT THE ROLE

This exciting new role will be part of our new Clinical Care Program. The aim of this program is to provide greater access to mental health services for those people living with mental illness in the Central Coast region. Mental Health Services in Primary Care will improve service integration and achieve alignment with a person centred, stepped care approach. The aims will be achieved by engaging a range of mental health professionals to provide a suite of evidence based mental health services to people that would otherwise have little or no access to such services.
In this role, you will work alongside the Specialist Lead and Support Worker to provide coordinated clinical care for people living with complex and enduring mental illness. You will guide the care that will be provided to clients and collaborate with the Support Worker to liaise with local networks in the community and provide the support required for each client on their mental health journey.
This is a permanent role based on the Central Coast and covers participants who on the Central Coast. CatholicCare is open to flexible working arrangements that meet the needs of the program.

KEY RESPONSIBILITIES:

  • Contacting clients within allocated timeframe in line with the Program’s needs.
  • Managing all aspects of the case management needs for the clients including holistic wrap-around supports.
  • Building relationships by developing a clear network of GP’s and Allied Health professionals to ensure the ongoing success of the program.
  • Supporting Specialist Lead in implementing and adapting the program as it evolves.
  • Commit to the Vision, Mission and Values of our organisation and active engagement in organisational events.
  • Ensuring compliance with incident management, reporting and escalation requirements and that organisational obligations under mandatory reporting, child protection, safeguarding and WHS are met.
  • Safety standards are maintained for self and others and all hazards and incidents are reported within appropriate timeframes.
  • Ensuring you have good working knowledge of mental health legislation and practices.

TO BE SUCCESSFUL IN THIS ROLE, YOU WILL HAVE:

  • Tertiary qualifications in a relevant area e.g.: Social Work, Welfare, Psychology, Health etc. or
  • Diploma level qualification with experience working in a Mental Health related area.
  • Ability to identify crisis situations and make sound decisions under pressure.
  • Knowledge of current Child Protection, Crimes (Domestic and Personal Violence) Act 2007 and other relevant legislation.
  • Experience in working in a client centred, trauma informed and culturally specific way.
