This is an exciting opportunity to join Torfaen CATCH (Clinical Assessment Team for Care Homes) . Torfaen CATCH nurses carry out clinical assessments (focused and comprehensive) for patients who reside in care homes throughout Torfaen and on behalf of GP Practices.
A Torfaen CATCH nurse will need to be passionate, proactive, and have a desire to gain new clinical knowledge and skills within the Frailty umbrella. They will be able to work flexibly and autonomously within a small team, have excellent understanding of patient deterioration and knowledge of onward referral procedures/community escalation processes to support health care closer to home/admission avoidance. The role will require advanced communication skills to facilitate advanced care plan discussions with patients and families, as well as to develop excellent working relationships with care home management and staff, the wider MDT and Community Resource Team.
You will be supported through a robust induction process.
The CATCH service currently runs on a Monday to Friday basis, not including Bank Holidays, covering the hours from 8 am to 18:30 pm
The post holder is expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills.
Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care.
The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues.
In particular they will be responsible for ensuring the timely provision of a high standard of nursing assessment, advice, planning, implementation, training support and evaluation of patient care.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
