JOB SUMMARY:

Oversee the running of the Senior Care Centre (SCC) and its services which include Day Care, Day Rehabilitation, Centre Based Nursing and Active Aging Programmes.

JOB DESCRIPTION

  • Perform intake assessment and review of day care clients and oversee Multi- Disciplinary Meetings for reviews of clients including Day Rehab clients.
  • Supervise daily routines such as meal preparation, serving of meals, preparing clients for activities, conducting of activities, reviews of clients and cleaning duties of care staff.
  • Work with centre nurse and care staff to ensure clients specific needs (ADLs) are met and communicated to the team.
  • Co-ordinate and plan programmes and services with internal and external stakeholders.
  • Direct, guide and participate in the orientation and training of new staff and provide ongoing training and monitoring of care staff in the area of provision of care to day care clients and clients with dementia.
  • Take charge of daily operations, and care provisions of clients, supervise and deploy staff to ensure operations of SCC is safe and effective.
  • Take charge of incidents in the centre related to staff, clients and volunteers to ensure that the incident is resolved, reported and follow-up.
  • Work with Centralised Team to ensure accurate billing and charges for clients and follow up on staff related matters.
  • Work with Cluster Lead to ensure financial sustainability of the centre.
  • Ensure accurate and timely documentation of communications, assessment and processes.
  • Carry out other responsibilities and duties as assigned from time to time.

JOB REQUIREMENTS

  • Recognized degree in a healthcare discipline, eg. nursing or allied health.
  • A minimum of 5 years’ experience in clinical area such as acute or community hospital,
  • Preferably at least 2 yrs in leadership position.
  • Good knowledge of the health care environment and service processes is important.
  • Experience in community care provision is an added advantage.
  • Integrity, highly motivated, a team leader and able to maintain good interpersonal relationships, in collaboration with various partners in the healthcare system.
