Children's Community Continuing Care Nursery Nurse

We are currently seeking experienced Nursery Nurses with good clinical and communication skills to join our service to work with Children and Young People in their own homes and other community settings. The post holder will join our forward thinking integrated work force supporting the provision of care with complex healthcare needs.
Applications are welcomed from experienced and newly qualified nursery nurses, as well as those wishing to return to practice. The caseload is varied but you will be well supported with a robust induction and support from your manager and colleagues, with regular safeguarding supervision.
You need to be responsive to changes in the delivery of care, demonstrate excellent interpersonal skills, support with safeguarding issues, and have a good understanding of children, young people, and health issues both locally and nationally.
All staff can access the essential to role training and professional developmental opportunities within Luton/Bedfordshire. You will need the ability to work on your own initiative but also as a team player. We are seeking nursery nurses who are motivated, innovative and enthusiastic.

Please note the post will be predominantly night shifts.
The successful candidate will need to be able to demonstrate the qualities in the person specification and be committed to their on-going professional development to ensure high standards.
The Children’s Continuing Care Nursey Nurse will provide high quality family centred nursing care for technology dependent children and children with complex health needs.
Care may be delivered in a range of settings appropriate to the children (e.g. school/home/respite unit) over the 24hr period.
The post holder will be required to work in partnership with children and young people with complex health needs, their families and the multi-disciplinary team to enable the child to attain the optimum life experience.
To build professional relationships with families who may be stressed and finding it difficult to cope with a child who has very complex health needs.
Communicate effectively and appropriately with children who have complex health needs and/or learning difficulties.
To carry out direct patient care as per the child’s daily care plan, including observations, medication administration and person cares (list not exhaustive).
To ensure a safe environment and the safe use of equipment and reporting any unsafe environment or unsafe use of equipment to the Team Leader.
To identify changes in the child’s condition assessing and referring appropriately to parents and/ or community nurse as part of escalation .

Clinical and Practice Governance
To perform nursing procedures as trained such as Tracheostomy care, administer enteral feeds, undertake suctioning (oral and tracheal), administer routine and emergency rescue medications
Follow emergency escalation frameworks and demonstrate an understanding of knowing how to manage an emergency situation, responding appropriately to ensure the child or young person remains safe
Follow a prescribed care plan
To use hoists and patient handling equipment as appropriate
To work in patients’ homes and other settings as required
Work in partnership with the community care nurse, Children’s Community Nursing Team, Special Needs School Nursing Team, children, families and other professionals to provide the optimum individualised nursing care in the most appropriate setting.
Meet daily therapy, hygiene and nutritional needs of the child.
Provide health education information to the child and family and carers to promote a healthy lifestyle as appropriate
Quality and IT
To keep accurate and contemporaneous records of care given
Responsible for own work with children as per written care plan.
To be involved in audits as requested by the team leader
To work within Evidence Based Practice and National Guidance to facilitate delivery of a high standard of care
To ask to be updated in the clinical nursing skills required for caring for very complex technologically dependent children
To participate in risk assessments as per Trust’s policy
To adhere to the Child Protection Policy
To follow recognised treatment and nursing policies
To work within the Trust’s policies, protocols and guidelines
To participate with the children’s continuing care nurses and other members of the team to develop and improve the servic
