Children's Community Nurse

Provide a community service to children requiring supporting nursing care in the community. Liaising with the wider multi disciplinary team and external agencies to support keeping patients at school and within the community. Facilitate early discharge and complex discharges Carry out regular oncology and cardiac community visits. Administer IV’s at home.
Administer chemotherapy in the community. Administer hormone injections. Support enteral fed patients. Carry out wound care assessments and dressings.
Hold a caseload of patients with long and short term health conditions. Provide 24/7 palliative care provision to families wishing to be at home at end of life. Working closely with local hospices and services to provide excellent care for the families. Educate and support colleagues.
Order supplies and equipment. Training opportunities available. Work closely with the inpatient and ambulatory settings along with specialist nurses and the medical team
