Good Shepherd Center is contracted by the Department of Health Services (OHS) to manage a Housing for Health (HFH) program which provides Intensive Case Management Services (ICMS) to women and families experiencing homelessness. Good Shepherd Center has three Scattered Site Housing Projects in which individuals are provided with housing resources to live independently in Los Angeles County. The case manager is responsible for providing direct services to unhoused single women and families throughout LA County, as part of the ICMS program. This includes home visits, accompaniments to medical appointments, housing application support, and resource linkage. This is a field-based position and reliable transportation is required.
Under the direction of the Housing for Health Program Manager, the Client Resources Coordinator Ill is responsible for providing individual case management to women and children experiencing homelessness utilizing a theoretical framework incorporating Trauma Informed Care (TIC), Motivation Interviewing (Ml), Critical Time Intervention (CTI) and Stages of Change (SoC). This framework provides tools that reflect best practices to motivate the participants to achieve their goals in a non- judgmental manner. The Case Manager will perform specialized tasks including comprehensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, counseling, individual benefit assistance, referrals and linkages to all Participants. The Case Manager will document all Participant interactions and input that data into the Homeless Management Information System (HMIS) and CHAMP databases.

QUALIFICATIONS:

  • Bachelor’s degree required; Master’s degree in a related field preferred.
  • 1-2 years of related experience.
  • One to two years of case management/client advocacy services with people who have been homeless and/or have a mental illness.
  • One to two years of experience utilizing evidence based practices in a social services field.
  • Valid California driver’s license
  • Ability to meet California minimum and Good Shepherds Center’s insurance requirement
  • Preferred Bilingual (English/Spanish)
  • Sensitivity and appreciation of diverse client populations as a benchmark to effectively to promote community and independent living skills

Responsibilities:

  • Provide individual case management, referrals and supportive services to Participants using a “Whatever it Takes Approach” to housing.
  • Maintain proper documentation and current case notes in computerized database systems ( HMIS & CHAMP).
  • Process housing applications with LA City and LA County Housing Authorities and provide support as the women transition into permanent supportive housing
  • Provide crisis management services to support clients’ housing retention, increase income and build community engagement.
  • Responsible for reporting abuse, as a mandated reporter which includes immediately reporting any concerns with respect to violation of a person’s rights, actual and/or suspected abuse
  • Present client case reviews to the Housing for Health Program Manager on a weekly basis.
  • Case Management caseload of 15-20 families and single adults
  • Develop relationships with housing providers, property owners, and property managers.
