Responsibilities:

YOUR ROLE

Play a pivotal role to support clients at the site location. Assist in the orientation and support of residents and day service users to the Coniston site, and work with the Illawarra team to support service users.

RESPONSIBILITIES

  • Provide a variety of supports to clients, working in co-operation with the Case Management team, volunteers and Conference.
  • Contact new and regular clients, providing support, information, referral to internal services and programs.
  • Be a presence and “contact point” for day service users and Residents to approach for support.
  • Liaison with appropriate service staff to ensure client needs are attended to in an effective and efficient manner and providing accurate, up-to-date information and practical support to clients on a range of issues.
  • Maintain client records, case notes and other documentation, both electronic and handwritten, in a professional manner.
  • Other duties related to the accommodation/support for clients as directed/required by site lead.
  • Welcome and orientate new residents to the service.
  • Provide a safe and welcoming environment for clients who live at or are accessing the service.
