JOB DESCRIPTION FOR CLINICAL ASSESSOR – HOMECARE

As Clinical Assessor you will play a critical role in the organisation by assessing the suitability of patients for receiving in-home care and treatment. You will work closely with MDTs you will evaluate medical conditions, the needs of the clients, and to determine what level of care is necessary for the individual client. Conducting comprehensive assessments to develop care plans and collaborate with MDTs maximising outcomes.

Responsibilities:

KEY RESPONSIBILITIES:

  • To Assess: all needs of the clients including medical, functional abilities, psychosocial for eligibility and suitability for home care services.
  • Risk Assess: against decision making, financial, environmental and medications risks.
  • Documentation: Maintaining accurate and updated assessment records, care plans, reviews and progress in a timely manner and in line with regulatory requirements. Updating system as required.
  • Reporting: on a weekly basis for managers and administrators.
  • Education: of both Home Support Workers and Clients to promote understanding of and adherence to the care plan.
  • Quality Assurance: participate in the overall quality improvement of the organisation. Identify possible improvements in enhancing home care delivery.
