EDUCATION AND EXPERIENCE:

  • Master’s Degree in Social Work, Psychology, Counseling, or related Human Services field, from an accredited college or university
  • MSW must have applicable DC licensure.
  • Two (2) years of direct service experience in a mental health setting with adults who have mental health issues.

SKILLS AND COMPETENCIES:

  • Ability to communicate in English, in written and oral form.
  • Skill in the use of computers, preferably in a PC, Windows-based operating environment and MS Office products; familiarity with Credible database.

Responsibilities:

  • Provide high quality services to consumers to include:
  • conducting annual psychosocial assessments using functional assessment tool for consumers served by assigned Community Support team.
  • leading the process of developing, updating, and guiding implementation of consumers Individual Recovery Plans (IRPs), in collaboration with consumer, Community Support Specialist (CSS) and Clinical Manager. IRP objectives will be specific, measurable, medically necessary and achievable within 180 days.
  • establishing constructive relationships with consumers, to promote acquisition of rehabilitative goals.
  • educating and assisting consumers with learning or re-learning Activities of Daily Living (ADL) skills necessary to improve the quality of life, including acquisition of consumer healthcare benefits.
  • identifying community resources to meet consumer’s identified needs and guide linkage of consumers to those resources.
  • acting as an advocate for consumers, to ensure adequate services are being provided to meet consumers’ needs.
  • Meet regularly for supervision with Clinical Manager and attend all administrative, in-service education and other meetings as required.
  • Must average 3.5 billable hours per day over a 12-month period.
  • Document billable clinical/ progress notes in Credible System. Documentation of all services provided within a day will be turned in by the next business day, following established procedures.
  • Update Consumer’s LOCUS, GAIN (DBH assessment tools) and Quarterly Data every 180 days.
