Just Better Care Central Coast are currently recruiting for the position of Clinical Care Coordinator to join our caring and dynamic office team, located in Gosford, Central Coast NSW on a part time basis.

Responsibilities:

ABOUT THE ROLE:

As a member of the Clinical team, the role of the Clinical Care Coordinator includes the provision of clinical oversight and support to ensure quality and safe clinical care and services to our customers within the scope of our service delivery obligations at Just Better Care Central Coast.
You will coordinate communication among numerous stakeholders, including customers or their families and/or representatives, the JBC clinical team, Customer Services and Supports team, NDIS Support Coordination, and Home Care Package Case Management teams, the People and Culture Team, referral partners, and industry bodies. Clinical experience is crucial, with an ideal preference for a minimum of 5 years experience in a comparable role.
This is a permanent part time role, 15.2 hours per week, spread over 2 or 3 days.

PRIMARY RESPONSIBILITIES:

  • Provide clinical oversight of service delivery.
  • Maintain high standard of clinical practice; including wound care, management of chronic conditions, specialised care, palliative support, disease prevention and health promotion.
  • Provide duty of care implementing WHS standards for customers and staff.
  • Perform Home Care Package and NDIS Health Assessments.
  • Ensure the development and review of Health Care plans for customers
  • Collaborate with key stakeholders, including JBCA head office, relating to clinical care requirements.
  • Work closely with the Leadership team to uphold clinical quality standards which align to the Aged Care and Disability standards.
  • Demonstrate commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policies and procedures, clinical governance framework, and legislative requirements.

PRINCIPAL ACCOUNTABILITIES

  • Participate in the intake and engagement processes for customers entering community care, particularly addressing identified clinical needs.
  • Ensure the assessment, planning, implementation, and evaluation of customer care align with relevant JBCA and Program guidelines.
  • Support the establishment of customer-focused systems and processes, emphasising consumer participation and a person-centred model of care, with a focus on clinical choice and control.
  • Supervise and hold accountability for clinical care, overseeing care plans, and conducting ongoing quality reviews of clinical documentation.
  • Implement and adhere to Clinical Quality, Infection Control, and Risk Management Programs.
  • Investigate and report incidents according to the Serious Incident Response Scheme.
  • Lead, motivate, coach, and mentor care staff to foster a high-performing and innovative workplace culture from a clinical perspective.
  • Assists Clinical Manager with staff education/training and support professional skill development.
  • Ensure team practices are informed by customer experience, regularly seeking and providing feedback.
  • Proactively identify opportunities for improvement to enhance team effectiveness and achieve positive customer outcomes.
  • Foster collaboration and strong working relationships to encourage shared goals and effective teamwork.
  • Drive strategic outcomes through innovative problem-solving, organisational skills, and process improvement.
  • Demonstrate and role model a commitment to self-development, openness to feedback, and resilience in challenging situations.
  • Ensure compliance with legislative requirements, Aged Care Quality Standards, and National Standards for Disability Services.
  • Provides direct care to customers, utilizing Registered Nurse (RN) skills as required.
  • Maintain personal professional skills, competence, and performance.
  • Fulfil other duties as required.
