Description:

Applications are invited from candidates for a 12-month Clinical Fellow post in Palliative Medicine, to commence in August 2024. The post is ideally suited to an individual who has satisfactorily completed Foundation Year 1 & 2 training and is considering a career in Palliative Medicine; and who wishes to gain experience in Specialist Palliative Care, including research, service improvement or teaching before commencing Core Specialty Training.
The Clinical Fellow will work primarily on the 12-bedded hospice in-patient unit where they will clerk in new patients, undertake independent and senior-led ward rounds and be a key part of the day to day running of the ward. Time on the in-patient unit is invaluable: it gives opportunities to learn about advanced symptom control and focus on communication skills with observation and supervised consultations.
The Fellow will contribute to the John Eastwood Hospice first on-call rota with the support of consultant colleagues. The rota is 1:6.5 and is non-residential. Part of being on call is to provide advisory telephone support to Bassetlaw Hospice inpatient unit, and the community and hospital services in mid-Nottinghamshire.
John Eastwood Hospice (JEH) was established in 1991. It is home to the Specialist Palliative Care (SPC) Service provided by Nottinghamshire Healthcare NHS Foundation Trust to the area of Mid Nottinghamshire, which has a population of around 300,000. The hospice is located next to Kings Mill Hospital, part of the Sherwood Forest Hospitals NHS Foundation Trust.
We have an in-patient ward with 12 beds: out-patient clinics, day hospice, community outreach team, and hospital team.
John Eastwood Hospice was purpose built with charitable money but is entirely run by the NHS. The building continues to be owned by the John Eastwood Charitable Trust with whom we have an excellent relationship. The partnership with the charity has allowed expansion of services and extension to the building.
Current Services
Inpatient unit
A 12 bed Inpatient Unit at John Eastwood Hospice, providing facilities for symptom management, assessment, and terminal care. The average length of stay is 12 days and 50% of patients are discharged to home or alternative place of care. A weekly MDT is held to discuss the in-patients. The proximity of the hospice to Kings Mill Hospital allows easy access to investigative procedures for hospice patients. Hospital Consultant colleagues visit the hospice to assess patients as required.
Outpatients
Outpatient clinics are held at John Eastwood Hospice and Newark Hospital.
Hospital Support Team
The hospital support team provide advice and support for patients in the Acute Hospital and Community Hospitals. The team consists of Clinical Nurse Specialists and Consultants. We hold a weekly MDT. We contribute to the training and development of palliative and end of life services at SFHT.
Community SPC Outreach Service
A Community MDT is based at John Eastwood Hospice and a further team at Newark Hospital. They work closely with primary care teams and hold weekly MDTs with Clinical Nurse Specialists, Consultant and training grade doctors, and other health professionals.
Non-malignant palliative care
Whilst most of the work remains cancer-related, there is an increasing commitment to non-malignant palliative care, including assessment and symptom management for patients with neurological conditions, heart failure and chronic lung disease
