Responsibilities:
MAIN DUTIES OF THE JOB
- Day to day operational management of virtual ward and virtual Care service working across admission avoidance services.
- Completion of required local and national performance reports
- Networking with wider admission avoidance system in Cornwall to support and enable new ways of working which support patients to receive care and support within their home environment as an alternative to hospital admission.
