About your opportunity
The Clinical Nurse Connected Care Program is dedicated to integrating and coordinating care providers for children and young people with complex and chronic health conditions across THHS.
Coordination of care is managed across the acute, community and primary health care sectors. If successful for this role you will be required to:
- Identify and create partnerships with members of the child’s health care team- including their GP, community and local care providers, hospital and healthcare providers and non-government organisations.
- Improve service access and provision for children with chronic and complex health needs by supporting optimal understanding of need, and the utilisation and integration of required health care services.
- Coordination of care through the provision of clinical assessment.
- Ensure families understand their child’s health care needs, so they can make informed health decisions.
Generous remuneration package value up to $132,289 p.a. comprising:
- Salary range of $107,960 to $115,604 p.a. (Nurse Grade 6 Band 1)
- Annual leave with 17.5% loading
- 12.75% employer contribution to superannuation.
