What’s involved - The purpose of this role is to review and redesign the establishment, implementation and expansion of Cancer services including Tele-Chemotherapy in Winton and Blackall. This will include consistent and effective clinical innovation practices and redesign processes to deliver solutions that are responsive to consumer needs, sustainable and enhance health services for this demographic. The role will work in collaboration with the Cancer Care Co-Ordinator CWHHS, external partners including Metro North Hospital and Health Service, visiting Oncologist, Townsville and Rockhampton Hospital and Health Service internal partners HUB facilities and Allied Health.
A cancer patients’ journey involves many health professionals in different treatment facilities across widespread locations. Patients and carers often need an experienced clinician to coordinate their care, to help them navigate health services, and to provide a single point of contact for information and support. The role of the Cancer Care Coordinator is designed to help fulfil this need. Cancer Care Coordinators provide continuity of care through diagnosis, treatment including surgery, chemotherapy and radiation therapy and follow-up.
