Responsibilities:
The Clinical Nurse Consultant (CNC) Community Palliative Care provides an advanced level assessment and management service as an expert in the specialty field and facilitates linkages and relationships with Local Health District (LHD) nurses and medical officers (in the acute and community settings) to support the management of community palliative care clients.
The CNC is responsible for the provision of expert clinical advice, coordination, leadership and consultancy services within the defined speciality and target populations within Integrated & Community Health (ICH), Western Sydney Local Health District (WSLHD).
The role includes assessment, care planning and review of clients in collaboration with primary service providers to ensure that evidence based practice is utilised.
