Responsibilities:
We have an exciting opportunity for a Nurse Extraordinaire who is passionate about health outcomes for patients presenting to the Emergency Department (ED) with Delirium.
This position will provide specialist assessment and evidence based care for residents of Residential Aged Care Facilities (RACFs), whilst working to investigate clinical pathways that may prevent avoidable presentations to the Emergency Department (ED) for delirium and improve delirium management within the community.
The position has key project deliverables working with stakeholders across Werribee Mercy Hospital, whilst also working in partnership with multiple RACF’s, the Victorian Virtual Emergency Department (VVED) and with other similar project roles across the West Metro Health Service Partnership.
You will work closely with the Hospital in the Home (HITH) Nurse Unit Manager and Health Independence Program (HIP) Manager to achieve the project goals, whilst also working with other clinical nurse consultants to provide expert clinical care to patients in Residential Aged Care Facilities. This role will suit an individual driven to improve care outcomes for older patients and is the perfect opportunity for a senior nurse who enjoys a mix of project work and a fast paced, dynamic clinical environment.
