At Monash Health Supportive and Palliative Care we are innovating palliative care for the residential aged care sector. The purpose of the position is to provide specialist palliative care clinical advice and care coordination to enquiries from aged care facilities via Residential InReach (RIR) and Hospital in the Home (HITH), under the framework of a research study.

Responsibilities:

ABOUT THE ROLE

Join our team as a Clinical Nurse Consultant on a Part Time (45 hrs per fn), 12-month Fixed term basis. The Clinical Nurse Consultant will provide day to day assistance and support to enquiries from aged care or Monash Health services involved in supporting aged care. This will include working closely with residential in reach and hospital in the home services. Your role will include data collection and data maintenance with the purpose of research, and you will participate in research activities such as research meetings and steering committee meetings.You will also.

  • Respond to enquiries from aged care via RIR, HITH or other Monash Health employees about palliative and supportive care matters and provide clinical advice, support, or care coordination.
  • Undertake informal palliative and supportive care education to aged care, RIR or HITH
  • Be able to read and understand a research protocol – foresee any difficulties and suggest solutions,
  • Contribute to the overall requirements of each protocol in conjunction with research team,
  • Review the study design, inclusion and exclusion criteria and verify all relevant source documentation in the subject’s medical record to confirm eligibility for the study,
  • Review the protocol, informed consent form, and follow up procedures,
  • Attend research-related meetings as appropriate, and
  • Communicate regularly with the research team regarding research related issues.

Secondary to your core role as a Palliative Care CNC you will also assist the clinical and research team in data collection for concurrent research undertaken in the department.You will also be able to demonstrate the following skills;
