Description:
The role of the Streaming CNM is to facilitate a safe streamlined pathways of care for patients from the emergency department and community settings who require acute medical assessment. They will ensure that multidisciplinary and multi-agency inputs are synchronised and targeted to match individual (acute and post-acute) care needs.
They will be a vital link within the Acute Floor setting for services in the Community (e.g. General Practitioners / Public Health Nursing) and actively link in with outside agencies such as Community Intervention teams in a timely manner to facilitate safe supported discharge.
